Many years have passed since the end of the second chapter.
An Earth spaceship approaches a strange asteroid.
The crew is led by Will, who is haunted by strange, very real dreams.
In the first he sees his own birth, passing in an instant from newborn to adult, but unable to leave the amniotic fluid.
At that moment he is making love with his assistant Terry, a woman genetically modified to obsessively seek sex.
She was supposed to be a prostitute, but Will gave her a new chance.
In the second dream Will meets Druuna, in a deserted and ruined city.
Druuna believes him to be a hallucination created by Lewis, but makes love to him anyway.
While Will tries to explain what happened to the ship's doctor (looking like Serpieri), Terry vents her instincts with the Proles, mutant beings used for heavy work and very similar to those infected with Morbus Gravis in the first chapters.
When they reach the surface of the asteroid, they find that it is composed of organic matter.
A chasm opens up on the surface, letting them through, but Will and Terry end up absorbed inside, losing their memory.
In that instant Druuna awakens from a lethargy induced by Lewis, which has kept her young.
Exploring the narrow corridors of the station, she reaches an inhabited area.
She is captured by Lornah and her adult children, who take her as a slave and broodmare.
The village they live in is surrounded by a giant mutated creature, which tries to absorb them.
Among the crowd of people who welcome her, Druuna sees Will, but does not seem to recognize her.
Upon reaching Lornah's house, Druuna becomes acquainted with Terry, with no memories of her previous life and forced to cater to Lornah's children.
Terry reveals her destiny to Druuna, which is to bear children conceived with Lornah's children.
Druuna would like to rebel, but is drugged by Lornah with an aphrodisiac and passionately indulges in all of them, including Lornah.
After the orgy she manages to escape with Terry and reaches Schastar, who commands the village.
Her lover has merged with Lewis and explains to her that the only way to save herself is to have sex with the creature, intent on reproducing.
However, Lewis stopped time on the spaceship, so no one can be born.
Lewis helps Druuna deal with coitus with the creature, making her believe she is making love to him.
The rest of the crew arrives soon after, rescuing Druuna, Will and Terry (who has forgotten what happened in the village).
Leaving the asteroid and reaching the shuttle, Druuna fears that the child of the creature is growing inside her.
