The story is set in the future, where Steve Rogers is old and no longer has the super soldier serum in his blood.
The United States is controlled by a fascist government, headed by James Stark, son of Tony Stark (Iron Man) and Janet Van Dyne (Wasp).
An incident from the past, in which super-heroes caused the destruction of Boston, made masked heroes illegal and allowed the establishment of the current government.
The accident was the result of a clash with Ultron, who empowered many super-villains and revealed the secret identities of the super-heroes.
When a documentary that wants to rehabilitate the Red Skull's memory is broadcast on TV, Steve opposes it.
Attacked after a TV interview, Steve is rescued by a group of masked rebels, The Defenders, led by Luke Cage, aka Power Man.
Steve agrees to undergo the Super Soldier Serum again, to stop the dictatorship.
James Stark, meanwhile, returns to visit his father, a prisoner in the Raft and now reduced to just a head kept alive artificially.
The premises seem interesting, even if they are a little too reminiscent of the legendary "Earth X" series by Alex Ross, who here only designs the cover.
